Rajasthan: How health department’s digital monitoring scheme is helping serve patients better

Rajasthan government’s ranking system of health services, Misaal, has started showing signs of on-ground impact on key public health indicators, say department officials.

Misaal issues the ranking of districts each month on the work done by the health department officials in their respective districts on the basis of mark secured by them in the three different categories including outcomes, intermediate outcomes and input or processes having weightage of 25%, 55% and 20% respectively.

“This is driving healthy competition across districts, who are now striving to drive ground-level innovation, which is then emulated by other districts,” an official with the department said. For example, Jhunjhunu has driven innovations in data collection processes, which have now been scaled-up across the state, he added. 

In order to drive effective and sustainable improvement of all key public health metrics, the department of health has developed a digital governance model, comprising of 3 key aspects: district rankings, auto-generated performance sharing at every level and digital feedback from the last-mile. 

The department officials attribute the increase in the state-wide “Full Immunization Coverage” from 65% to 78% in 3 months (May to July, 2018) to Misaal.    

It’s RajDharaa mobile application that enables facility inspections by health officers has resulted in the improved attendance of medical officers by 30 per cent after 400 visits, officials said. 

In the latest ‘Misal’ ranking for the month of July, Sikar is at the top with 49.47 score followed by Jhunjhunu, Chittorgarh and Sriganganagar. Sikar had secured the first place in the list for the month of May as well when the ranking was issued for the first time. Hanumangarh is also among the top 10 performing districts. The district has shown the best improvement in the rankings by reaching number 8 in August from the 17th rank in July.

“Traditionally, information sharing has been a tedious process in public health in India. Rajasthan is the first state to innovate how data is being digitally shared with every cohort, to enable a more data-driven culture across the system,” an official said. Digital data sharing is catalyzing a fundamental shift in the way the public health system operates, say officials. Officers at every level (such as District and Block) have begun analyzing this data every month to assess which areas their district/ blocks are lagging in and what changes they need to introduce. “We truly believe that via this governance model, Rajasthan can be a “misaal” for the nation, when it comes to public health transformation,” the official added.
